Here at Fellowship Square Surprise, the safety and well-being of our residents, staff, and all families is our highest priority. From the beginning of the COVID-19 pandemic, we have continued to closely follow guidelines set forth by the CDC, AZDHS, and Maricopa County Department of Public Health.

Guidance for Assisted Living Settings
Masks are still required in Assisted Living, regardless of vaccination status. Additionally, Assisted Living visitors and staff will still be required to check in and complete a health screening before entry.
It is important to note, that these policies are subject to change with or without notice, should an outbreak of COVID-19 reoccur, or updated guidelines change, further restrictions may be reinstated.
