Three Levels of Assisted Living Services

The Oasis at Fellowship Square Surprise offers three levels of assisted living services: Essential, Enhanced, and Elite. To ensure every resident receives the support they need, we start with a one-on-one assessment with our nurse before admission. Once the appropriate service level is determined, the nurse will review the care plan with the resident and their representative upon admission. Residents can receive skilled nursing services from a home health agency or hospice care.

Essential Services

Also known as Level 1, our certified caregivers offer light support with daily activities, encourage socialization, and perform daily wellness checks and medication reminders.

Residents receiving Essential Services should be able to ambulate independently, manage personal hygiene tasks such as bathing, toileting, and grooming, express their needs effectively, and make safe decisions.

Enhanced Services

Also known as Level 2, Residents receive assistance with daily care activities such as bathing, dressing, and grooming. Our staff also assists with ambulation, transferring, medication management, and administration.

Residents receiving Enhanced Services must be able to eat independently, request assistance, and communicate their care needs effectively.

Elite Services

Also known as Level 3, our certified caregivers provide increased support with daily activities, including walking, transferring, bathing, toileting, dressing, selecting meals in the dining area, and medication administration.

Residents receiving Elite Services may experience challenges recognizing potential hazards, requesting help, or making simple care-related decisions.

How Assisted Living Pricing Works

Our Assisted Living pricing includes two components: a monthly base rate for the apartment and the rate for the level of care services. This approach allows residents to choose the apartment that best fits their lifestyle while receiving the level of support that best meets their needs. 

To put it simply: Apartment Base Rate + Level of Services = Total Monthly Investment.

Your monthly base rate depends on the apartment you choose and includes a range of services and amenities that support a comfortable, connected lifestyle. From dining and housekeeping to engaging programs and shared spaces, many everyday essentials are included.

Assisted Living
Prices & Floor Plans

We offer studio and one-bedroom/one-bathroom Assisted Living apartments. All units feature high ceilings, a kitchenette, a spacious bathroom, and a walk-in shower with safety bars.

Monthly Rates for Assisted Living Services

Each resident begins with a personal assessment by our nurse to help us understand their unique needs. From there, a monthly service plan is determined, and that rate is added to the base rate of their chosen apartment, ensuring the right level of support is always in place.

Second Occupant Monthly Rates

Base Rate: $750/month + Level of Care
