Subsidized Assisted Living for Seniors

Christian Care Manor II

At Christian Care Manor II, we provide Assisted Living for low-income seniors, age 62 and older.

We are licensed to provide three levels of Assisted Living Services: Supervisory, Personal and Direct care. Please scroll down to read complete details on each level of care.

Our programs include three meals a day in a full-service dining room, planned social activities and certified caregivers on staff 24 hours a day. Each resident wears an emergency call button which, when activated, alerts the caregivers that the resident needs help.

Paying for Assisted Living

Navigating the wide range of senior living options can be difficult. For seniors living on a low income, the added stress of figuring out how to pay for Assisted Living can be overwhelming.

Admission Qualifications for HUD-subsidized Senior Housing

  1. The household's annual income may not exceed the very-low income limits set by the U.S. Dept. of HUD for the present year. For 2021, the very low income limits set by HUD are $27,650 for a single person, $31,600 for couple.
  2. The applicant agrees to pay the rent required by the subsidiary program under which the applicant will be admitted. (Residents typically pay 30% of their adjusted income for rent and utilities).
  3. The unit will be the family's only residence.
  4. Residents must be 62 years of age or older.

Arizona Long Term Care System (ALTCS)

In Manor II, we accept both private pay and ALTCS benefits. ALTCS is the State of Arizona’s Medicaid program that provides long term care services for individuals who are 65 or older, blind, or disabled and need ongoing skilled nursing services. 

We work with all three ALTCS plan providers: United Heath Care, Mercy Care, and Banner University Heath Plans.

If you or your loved one are searching for Assisted Living and think you might qualify for ALTCS assistance, we strongly encourage you to begin reviewing the program requirements and complete a 'Request for Application for ALTCS' as soon as possible. The application process can take anywhere from 6 weeks to as long as 6 months.

Levels of Care Explained


LEVEL 1 – Supervisory Care

In Supervisory Care, our certified caregivers provide some assistance with activities of daily living, including meal preparation, housekeeping and laundry. Residents receiving Supervisory Care services must be able to ambulate without staff assistance, toilet, bathe & dress independently, as well as be able to communicate needs and make safety decisions appropriately.

LEVEL 2 – Personal Care

In Personal Care, our certified caregivers provide moderate assistance with activities of daily living, such as grooming and dressing, incontinence management and medication reminders. Residents receiving Personal Care services must be able to summon assistance and communicate care needs to staff.

LEVEL 3 – Direct Care

In Direct Care, our certified caregivers provide a higher level of assistance with activities of daily living such as ambulation, bathing, toileting, dressing, selecting menu items in the dining room and medication management. Residents in Directed Care are sometimes unable to recognize danger, summon assistance or make basic care decisions.
